We pride ourselves on working with individuals and families to deliver the best possible service. If you are interested in beginning the journey towards greater independence, then please get in touch for a no obligation chat.
The vast majority of the people we support have a personal budget which enables them to purchase our services and we can guide you through this process.
From the initial referral for a community care assessment – we’re with you every step of the way

How we help

We guide people through the journey to greater independence, working with a variety of other agencies to assist you with every step of the transition from living in the family home, to a home of your own – ensuring the best possible outcome.

Initially, we would arrange an informal chat to establish your needs and preferences, following which our experienced team will identify an action plan. We can offer help and guidance at each stage and will provide as much support as each individual requires; whilst we actively encourage the continued involvement of service users and their families, we can also liaise with external agencies on your behalf.

We can assist with:

Community care assessments

Conducted by social workers to determine an individuals needs, this then generates a personal budget which enables people to purchase appropriate support to meet their needs. We can work with you to ensure all needs are correctly identified and appreciated, and then reflected in an appropriate care package.

Person-centred support planning

Each person we support has a detailed support plan which provides information on how best to support that person. From which football team they support to how they like their morning cup of tea, our support plans put the individual at the centre of everything we do, detailing what is important to and for the people we support. Everyone is different. Support plans are reviewed regularly and the involvement of family and friends is encouraged to ensure that we deliver the best possible support which reflects the individual’s goals and aspirations.

Finding a property and maintaining a tenancy agreement

We work with Registered Social Landlords to enable individuals to find their ideal accommodation, ranging from smart town centre apartments within secure complexes, to peaceful bungalows with beautiful gardens, and everything in between… Once you’ve found your ideal home, we provide support to ensure individuals are able to fulfil the terms of their tenancy agreements. We can also assist with grant applications for help with the cost of any necessary adaptations, specialist equipment, etc.

Benefit advice

As many of the people we support are in receipt of benefits, we can offer assistance to ensure that they maximise their income and receive all the help to which they are entitled. We can also arrange for help to complete the forms.

Healthcare professionals

Some of the people we support have a range of complex health needs; we work with others including district nursing teams, GPs, podiatrists and occupational therapists to ensure that all a person’s needs are appropriately met and their health and well-being is maximised.
